CHI St. Alexius Health Dickinson (N.D.). CHI St. Alexius Health Dickinson, formerly CHI St. Joseph's Health, became part of the Englewood, Colo.-based Catholic Health Ini- tiatives network in 1996. It was combined with several other CHI hospitals in the area to form CHI St. Alexius Health in April 2016, a regional system. CHI St. Alexius Health Dickinson, a 25-bed critical access hospital, is fully accred- ited by the Joint Commission and offers a full range of healthcare services to its surrounding communities, including a Level IV trauma center. In 2015, the hospital earned a Superior Performance in Safety Improvement award and a Quality Improvement Effort Honor in Obstet- rics from the Minnesota Hospital Association. Delta (Utah) Community Hospital. Delta Community Hospital, formerly known as Delta Community Medical Center, joined Salt Lake City-based Intermountain Healthcare in 1980. e 18-bed critical access hospital has under- gone extensive renovations to improve patient rooms and women's and newborn areas. is year, DCH was recognized as a Top 100 Critical Access Hospital by iVantage Health Analytics. Fairview Hospital (Great Barrington, Mass.). Fairview Hospital is owned by Pittsfield, Mass.- based Berkshire Health Systems. e hospital has garnered a string of awards in recent years for its commitment to patient safety and quality care. It achieved a Top Rural Hospital desig- nation by e Leapfrog Group in 2015 — an honor it has maintained for four consecutive years — a Healthgrades Outstanding Patient Experience Award in 2016 and a five-star rating from CMS. Linked to specialists at Berkshire Medical Center through telemedicine, the Mas- sachusetts Department of Health has designat- ed Fairview as a primary stroke center. Floyd Valley Healthcare (Le Mars, Iowa). Floyd Valley Healthcare is a municipally owned facility that includes a critical access hospital, three family medicine clinics and an assisted living facility. It is affiliated with the Avera Network in Sioux Falls, S.D. FVH was named a Top 20 Critical Access Hospital by the National Rural Health Association as well as a Top 100 Critical Access Hospital by iVantage Health Analytics in 2016. FVH was also recognized as a 2016 Healthgrades Outstanding Patient Ex- perience Award recipient and is among the top six hospitals in Iowa with the cleanest rooms, as reported by patients on HCAHPS surveys. Gunnison (Colo.) Valley Hospital. Gunnison Valley Hospital is a community-owned, inte- grated health system that provides a full range of healthcare services. GVH's dedication to excellence in patient care has caught the eye of national healthcare organizations. For instance, iVantage Health Analytics recognized GVH as a Top 100 Critical Access Hospital in America for five consecutive years, most recently in 2016. GVH also earned distinction from iVan- tage as a HealthStrong Hospital in 2015. Hancock County Hospital (Sneedville, Tenn.). Hancock County Hospital, part of Kingsport, Tenn.-based Wellmont Health System, opened in 2005 to provide healthcare services to the Sneedville community. In 2015, HCH was named a Top 20 Critical Access Hospital by the National Rural Health Association for patient satisfaction. In 2012, it was named a Top 20 Critical Access Hospital by NRHA for quality. Hammond-Henry Hospital (Geneseo, Ill.). Hammond-Henry Hospital, located about 150 miles west of Chicago, was founded in 1901. Hammond-Henry Hospital's emergency depart- ment was named a 2015 Guardian of Excellence Award recipient by Press Ganey. e award honors organizations within the 95th percentile for patient experience, employee or physician engagement, or clinical quality performance. e hospital demonstrates excellence in patient expe- rience in other ways: is year, Hammond-Hen- ry Hospital was one of just 328 nursing centers to earn Providigm's Embracing Quality Award for customer satisfaction in 2015. Hudson (Wis.) Hospital & Clinic. Hudson Hospital, part of Bloomington, Minn.-based HealthPartners, demonstrates excellence in pa- tient care and environmental efforts. Hudson's Birth Center nursing staff ranks among the top 3 percent nationwide, according to Press Ganey patient survey data. is year, Hudson earned several awards from Practice Greenhealth for outstanding performance in healthcare sustain- ability, including recognition for food services and operating room sustainability. Huggins Hospital (Wolfeboro, N.H.). For more than 100 years, Huggins Hospital has provided healthcare services to residents and visitors of the Eastern Lakes region. Today, this 25-bed, nonprofit critical access hospital offers primary, secondary and specialty care at its main campus in Wolfeboro and at four primary care locations in Alton, Moultonborough, Ossipee and Tamworth. Huggins Hospital was named one of the Best and Brightest Compa- nies to Work For in the country by the National Association for Business Resources. It was also named a 2016 Top 20 Critical Access Hospital for Quality by iVantage Health Analytics. Kittitas Valley Healthcare Hospital (Ellens- burg, Wash.). Situated in the heart of Washing- ton, Kittitas Valley Healthcare Hospital provides quality healthcare services to residents of Kittitas County, which roughly 41,000 people call home. KVH has a vested interest in community out- reach and holds community roundtables to fos- ter engagement in regional health initiatives and wellness programs. KVH Hospital was ranked among the Top 100 Critical Access Hospitals in the nation by iVantage Health Analytics in 2016. It is one of just 16 hospitals named to iVantage's list more than five times. Madison (S.D.) Regional Health System. Madison Regional Health System, formerly known as Madison Community Hospital, operates a 22-bed nonprofit, independent healthcare facility that has served Madison and its surrounding communities for more than a century. e hospital was named among the Top 100 Critical Access Hospitals in the nation for the past three consecutive years, by iVan- tage Health Analytics. It won the 2015 Growth and Achievement Award from the Lake Area Improvement Corporation for its positive economic impact on the community. Magruder Hospital (Port Clinton, Ohio). Ma- gruder Hospital offers quality healthcare ser- vices to the Port Clinton community, about 44 miles east of Toledo, Ohio. is 25-bed hospital became a federally certified critical access hos- pital in December 2002 and has since earned e Joint Commission's Gold Seal of Approval. Magruder was named a Top 100 Critical Access Hospital in 2016, as well as a 2015 Health- strong Hospital, by iVantage Health Analytics. It boasts a five-star rating from CMS based on patient data from the HCAHPS survey. Martha's Vineyard Hospital (Oak Bluffs, Mass.). Since its beginning in 1922, Martha's Vineyard Hospital has offered the Oaks Bluff community high quality, community-focused medical care. MVH became a member of Bos- ton-based Partners HealthCare and an affiliate of Massachusetts General Hospital in April 2007, enabling access to advanced specialty care and physician expertise from some of the nation's premier healthcare institutions. Due to its environmental design, the MVH facility was awarded a Leadership in Energy and Environmental Design GOLD certification in 2011. It has been named on iVantage Health Analytics' Top 100 Critical Access Hospitals list for the past three years. Mayo Clinic Health System-Red Cedar (Menomonie, Wis.). Mayo Clinic Health Sys- tem-Red Cedar is a physician-led, integrated sys- tem serving families in western Wisconsin. Red Cedar's medical clinic and hospital merged with Rochester, Minn.-based Mayo Clinic Health System in 1995 and 1996, respectively, creating a multispecialty medical and surgical group. For the fih consecutive year, Mayo Clinic-Red Cedar was included in the Top 100 Critical Access Hospitals by iVantage Health Analytics in 2016. It also garnered a Patient Safety Excellence Award from Healthgrades this year.
