Located in the Des Moines River valley in North- west Iowa, Avera Holy Family Hospital was estab- lished in 1944. In 2015, iVantage Health Analytics recognized Avera Holy Family Hospital as both a HealthStrong Top Hospital and a Top 100 Critical Access Hospital. The hospital also earned a five- star rating on its HCAHPS survey, one of just 251 hospitals nationwide to do so. Banner Boswell Medical Center (Sun City, Ariz.). In 2015, Banner Boswell Medical Center was named a Truven 100 Top Hospital and an iVantage HealthStrong Top Hospital. The 501- bed hospital located approximately 25 miles northwest of Phoenix was also recognized by Healthgrades with a Patient Safety Excellence Award and a Stroke Care Excellence Award in 2015. U.S. News & World Report ranked Ban- ner Boswell Medical Center among the top 15 hospitals in the Phoenix metropolitan area and Arizona in its 2014-15 rankings. Beverly (Mass.) Hospital. Beverly Hospital is a 221-bed hospital located on the North Shore of Massachusetts, roughly 25 miles from Boston. With a medical staff of more than 500 physi- cians, the hospital serves roughly 13 communi- ties. Beverly Hospital is a seven-time Truven 100 Top Hospital and a 2015 iVantage HealthStrong Top Hospital. CareChex ranked the hospital No. 14 in the state in overall hospital and surgical care and No. 3 in overall medical care in 2015. U.S. News & World Report rated Beverly Hospital high performing in heart failure, hip replace- ment and knee replacement in 2014-15. Blanchard Valley Hospital (Findlay, Ohio). Blanchard Valley Hospital has 150 beds and had served an eight-county area for more than 100 years, as it was founded in 1891. It serves as the flagship of Findlay-based Blanchard Valley Health System. The hospital was named a HealthStrong Hospital by iVantage Health Analytics this year and was also named as one of the 100 Top Hos- pitals in the nation by Truven Health Analytics. Additionally, the hospital received three Health- grades awards this year, including the Distin- guished Hospital Award for Clinical Excellence and the Outstanding Patient Experience Award. Bozeman (Mont.) Deaconess Hospital. Located in the heart of the Rocky Mountains in southwest Montana, Bozeman Deaconess is a Joint Commis- sion certified, licensed Level II trauma center. In 2015, the 86-bed hospital was recognized with an A Hospital Safety Score by The Leapfrog Group for the second time in a row and the fifth time overall. In 2015, Bozeman Deaconess was named an iVantage HealthStrong Top Hospital and one of America's 100 Best for Pulmonary Care by Healthgrades. The hospital also earned a four-star patient experience score on its HCAHPS survey. Brodstone Memorial Hospital (Superior, Neb.). Brodstone Memorial Hospital is a 25-bed facility located just a couple of miles from the Nebras- ka-Kansas border. Brodstone was named both a HealthStrong Top Hospital and a Top 100 Criti- cal Access Hospital by iVantage Health Analytics. Also in 2015, Healthgrades recognized the hos- pital with the Outstanding Patient Experience Award for the second year in a row. Brodstone has a five-star HCAHPS rating. Buffalo (Minn.) Hospital. Located on the west- ern edge of the Twin Cities metropolitan area, this hospital has 65 licensed beds and employs 432 people. Buffalo Hospital was born out of a three-room medical practice above a hardware store that opened in 1905. In 2015, Buffalo Hos- pital was named a Truven 100 Top Hospital and an iVantage HealthStrong Top Hospital. Also this year, Healthgrades recognized the facility with a Patient Safety Excellence Award and CareChex ranked Buffalo Hospital No. 21 in the state of Minnesota for overall hospital care. Calumet Medical Center (Chilton, Wis.). Calu- met Medical Center — part of Menasha, Wis.- based Affinity Health — is roughly 40 miles south of Green Bay and is the only hospital in Wiscon- sin's Calumet County. The medical center is both a 2015 HealthStrong Top Hospital and a Top 100 Critical Access Hospital, as designated by iVantage Health Analytics. Calumet Medical Center also earned a five-star rating on its HCAHPS survey. Cape Cod Hospital (Hyannis, Mass.). With rough- ly 85,000 annual patient visits, Cape Cod Hospi- tal's emergency department is one of the busiest in Massachusetts. The roughly 300-bed hospital is accredited by the Joint Commission, the College of American Pathologists and the American Associa- tion of Blood Banks. In 2015, Cape Cod Hospital was named an iVantage HealthStrong Top Hospi- tal and received the Distinguished Hospital Award for Clinical Excellence and six specialty excellence awards from Healthgrades. Carolinas Medical Center-NorthEast (Concord, N.C.). In its 2014-15 rankings, U.S. News & World Report named Carolinas Medical Center-North- east the No. 8 hospital in North Carolina and the No. 2 hospital in the Charlotte metropolitan area. The 457-bed medical center is an iVantage 2015 HealthStrong Top Hospital. Also in 2015, Healthgrades recognized Carolinas Medical Cen- ter-Northeast with the Patient Safety Excellence Award and specialty excellence awards in stroke care, pulmonary care and neurosciences. CaroMont Regional Medical Center (Gastonia, N.C.). CaroMont Regional Medical Center is the flagship of CaroMont Health, a system with five af- filiate companies including a network of 46 primary and specialty physician offices. The 435-bed medical center was named a HealthStrong Top Hospital by iVantage and one of America's 100 Best Hospitals for spine surgery by Healthgrades in 2015. U.S. News & World Report named CaroMont Regional Medical Center the No. 3 hospital in the Charlotte, N.C., metropolitan area in its 2014-15 rankings. Centinela Hospital Medical Center (Inglewood, Calif.). The only hospital located in Inglewood, Calif., Centinela Hospital Medical Center serves more than 140,000 patients and their families from Los Angeles and the South Bay regions each year. U.S. News & World Report named Centinela the No. 25 hospital in California and the No. 13 hospital in the Los Angeles metropolitan area in its 2014-15 rankings. Centinela is also fully ac- credited by the Joint Commission, ranked a 100 Top Hospital by Truven Health Analytics and the recipient of numerous excellence awards from Healthgrades. Central DuPage Hospital (Winfield, Ill.). North- western Medicine Central DuPage Hospital is a nationally recognized, 333-bed facility with more than 1,040 physicians with focuses in roughly 90 specialties, including oncology, neurology, or- thopedic, pediatric and cardiology care. In 2014, CDH was one of only 94 hospitals in the nation to be named as a Top Hospital by The Leapfrog Group. In 2015, it was named a 100 Top Hospital by Truven Health Analytics for the fourth consec- utive year and the ninth time total. Healthgrades recognized CDH with the Outstanding Patient Experience Award as well in 2015. Cheshire Medical Center/Dartmouth-Hitch- cock Keene (N.H.). Located in Southwestern New Hampshire, between Boston and Hartford, Conn., Cheshire Medical Center is a 169-bed re- gional referral center joined with medical practice Dartmouth-Hitchcock Keene, which boasts more than 125 providers. In 2014, CMC/DHK was ac- credited by The Joint Commission. In 2015, iVan- tage Health Analytics named Cheshire Medical Center HealthStrong Top Hospital. Cheyenne (Wyo.) Regional Medical Center. Cheyenne Regional Medical Center — located in Wyoming's capital — is nearly 150 years old and is home to the state's first Level II trauma center. The 222-bed medical center includes a medi- cal staff of more than 170 clinicians and more than 2,000 employees. In 2015, CareChex ranked Cheyenne Regional Medical Center No. 1 in the state for overall hospital, medical and surgical care, as well as in numerous specialties. CHI Health St. Francis (Grand Island, Neb.). St. Francis Medical Center's history dates back to 1887 when the Sisters of Saint Francis opened a hospital in Grand Island. It has grown through the years into a regional referral center with roughly 100 physicians and more than 1,100 em- ployees and is part of Omaha, Neb.-based CHI Health. Not only is CHI Health St. Francis com- mitted to providing a great patient experience — the hospital earned the Outstanding Patient Experience Award from Healthgrades in 2013 and 2014 — it is also committed to the environment: CHI Health St. Francis was awarded the 2015 Greenhealth Partner for Change award by Prac- tice Greenhealth this year. Columbus (Ind.) Regional Hospital. Colum- bus Regional Hospital is the 225-bed flagship of Columbus Regional Health, a system that serves a 10-county region in southeastern Indiana. The hospital was also Indiana's first Magnet-desig- nated hospital for outstanding nursing care. In 2015, Columbus Regional Hospital was named an iVantage HealthStrong Top Hospital and received the Outstanding Patient Experience Award from Healthgrades.
