This critical access hospital has served the Anaconda- Deer Lodge County and Southwest Montana since 1889. Community Hospital of Anaconda earned numerous accolades in 2015, including being named a HealthStrong Top Hospital by iVantage. CHA was also named a Top 100 Critical Access Hospital for the fifth year in a row in 2015. Care- Chex ranked CHA within the top 10 hospitals in the state in overall hospital and surgical care this year and the Mountain Pacific Quality Health Foundation recognized the hospital with the High- est Quality Award for the tenth consecutive year. Community Memorial Hospital (Menomonee Falls, Wis.). Community Memorial Hospital was founded by the citizens of Menomonee Falls, Wis., in 1964. Today, this 202-bed, Joint Commis- sion-accredited hospital serves residents across four counties in the Greater Milwaukee area. The facility provides comprehensive inpatient and outpatient services in more than 51 medical spe- cialties. Community Memorial Hospital is a 2015 iVantage HealthStrong Top Hospital. Cookeville (Tenn.) Regional Medical Center. Es- tablished as a 15-bed private clinic in 1921 and pur- chased by the city of Cookeville in 1927, the medical center has grown considerably over the years. To- day, Cookeville Regional is comprised of 247 beds, 150 physicians and more than 2,000 employees, In 2015, for the third year in a row, CRMC was ranked among America's 100 Best Hospitals for cardiac care, coronary interventional and orthopedic surgery by Healthgrades. The medical center is also an iVantage HealthStrong Top Hospital. Cottage Grove (Ore.) Community Medical Center. Cottage Grove Community Medical Center is part of Vancouver, Wash.-based PeaceHealth. Located about 130 miles from Portland, Ore., the 14-bed facility offers numerous medical services, such as physical therapy, outpatient services and imaging services, as well as support services, such as end-of- life care, home health and hospice and pastoral and spiritual care services. Cottage Grove Community Medical Center was named an iVantage Health Ana- lytics' HealthStrong Hospital this year. Delray Medical Center (Delray Beach, Fla.). As both a community hospital and a Level I trauma center, Delray Medical Center serves the commu- nities of Florida's South Palm Beach County. The 493-bed hospital employees has earned numer- ous recognitions in 2015, including being named a Truven Health 100 Top Hospital and earning an "A" Hospital Safety Score from the Leapfrog Group. Additionally, Healthgrades named Delray one of America's 50 Best Hospitals for the ninth consecutive year and recognized the hospital with the Distinguished Hospital Award for Clinical Ex- cellence for the thirteenth consecutive year. Desert Valley Hospital (Victorville, Calif.). This 148-bed hospital was founded and built in 1994 approximately 8 miles northeast of Los Angeles, in southwestern San Bernardino County. It re- ceived a coveted "accreditation with commenda- tion" designation from the Joint Commission in its first year of operation. In 2015, Desert Val- ley Hospital was named a 100 Top Hospital by Truven Health Analytics and received the Distin- guished Hospital Award for Clinical Excellence and the Patient Safety Excellence Award from Healthgrades. Dublin (Ohio) Methodist Hospital. Dublin Meth- odist has roughly 60 patient beds and is located in a suburb of Columbus, Ohio. The hospital of- fers a full service emergency department, women's health, cardiac imaging and an anticoagulation clinic, as well as a variety of outpatient and inpa- tient surgical services. In 2015, Dublin Methodists was named a 100 Top Hospital by Truven Health Analytics for the fourth consecutive year and re- ceived Healthgrades' Outstanding Patient Experi- ence Award for the third consecutive year. East Alabama Medical Center (Opelika). Estab- lished in 1952 as Lee County Hospital, the hos- pital changed its name to East Alabama Medical Center in 1981. Today, EAMC — located in the principal city of the Auburn-Opelika Metropoli- tan Area in the east central part of Alabama — is licensed for 314 general, acute care beds and houses a 26-bed skilled nursing facility. In 2015, iVantage named EAMC a HealthStrong Top Hos- pital and Healthgrades recognized the medical center with two patient safety awards and three specialty excellence awards. Eau Claire (Wis.) Hospital. Eau Claire Hospi- tal was originally founded as Luther Hospital in 1908. Today, the 304-bed hospital is part of the Rochester, Minn.-based Mayo Clinic Health Sys- tem and boasts the only verified trauma center in northwest Wisconsin. Eau Claire Hospital is a 2015 iVantage HealthStrong Top Hospital and a three-time recipient of the Healthgrades' Out- standing Patient Experience Award. U.S. News & World Report named the hospital the No. 6 hospi- tal in Wisconsin in its 2014-15 rankings. Elizabethtown (N.Y.) Community Hospital. Located in northern New York, nearly 30 miles southwest of Burlington, Vt., Elizabethtown Community is a Joint Commission-accredited critical access hospital. Its emergency department treats about 5,500 patients each year, nearly one- third of which are trauma cases. In 2015, iVantage recognized it as both a HealthStrong Top Hospital and a Top 100 Critical Access Hospital. This 25- bed facility boasts 194 employees, including 18 medical staff providers and more than 25 cour- tesy and consulting physicians. Elmhurst (Ill.) Memorial Healthcare. Originally founded in 1926, Elmhurst Memorial Hospital changed its name to Elmhurst Memorial Health- care when it expanded its services. Now, the 259-bed hospital provides comprehensive services to patients throughout Chicago's western suburbs. In 2015, Elmhurst Memorial Healthcare received excellence awards in patient safety and general surgery from Healthgrades. U.S. News & World Report ranked El- mhurst among the top 20 hospitals in the Chicago metro area and Illinois in its 2014-15 rankings. Fairfield (Ohio) Hospital. Fairfield Hospital serves patients in Butler County and is part of Cincinna- ti-based Mercy Health. The 192-bed hospital was named one of the 100 Top Hospitals and one of the 50 Top Cardiovascular Hospitals in 2014 by Truven Health Analytics. In 2015, iVantage named Fairfield a HealthStrong Top Hospital and Health- grades recognized the hospital with awards in pa- tient safety and patient experience, overall clinical excellence and excellence in five specialties. Fairview Hospital (Great Barrington, Mass.). Though it only has 25 beds, Fairview Hospital of- fers a full range of primary and diagnostic services. The hospital has received numerous accolades, in- cluding being named a Top Rural Hospital by The Leapfrog Group in 2014 for the third consecutive year. In 2015, Fairview Hospital was named an iVan- tage HealthStrong Top Hospital and received a pa- tient safety and patient experience excellence award from Healthgrades. The critical access hospital also earned a five-star rating on its HCAHPS survey. Fort Madison (Iowa) Community Hospital. The list of Fort Madison Community Hospital awards and accolades is lengthy. In 2015, the 50-bed hos- pital was named a Truven 100 Top Hospital and an iVantage HealthStrong Top Hospital. Also, Care- Chex ranked Fort Madison within the top 20 hos- pitals in the state in overall hospital, medical and surgical care. This Joint Commission-accredited facility is located roughly 100 miles southwest of Davenport, Iowa. French Hospital Medical Center (San Luis Obis- po, Calif.). Situated roughly midway between Los Angeles and San Francisco, French Hospital Med- ical Center has served San Luis Obispo County for nearly 70 years. The hospital's namesake, Dr. Edi- son French, claimed to be the first surgeon in San Luis Obispo to use intravenous anesthesia, the first to perform a lung resection and the first to do a collapsed lung therapy. Truven Health Ana- lytics named French Hospital Medical Center one of its 100 Top Hospitals in 2015. Also this year, the hospital received Healthgrades' Patient Safety Ex- cellence Award, as well as three Healthgrades spe- cialty excellence awards for gastrointestinal care, pulmonary care and joint replacement. Garden Grove (Calif.) Hospital Medical Center. Garden Grove Hospital Medical Center, a subsid- iary of Ontario, Calif.-based Prime Healthcare Services, is a 167-bed hospital located more than 30 miles south of Los Angeles. Garden Grove Hospital Medical Center was named a 100 Top Hospital by Truven Health Analytics in 2015. The hospital also received the Patient Safety Excel- lence Award from Healthgrades this year. Good Samaritan Hospital (Vincennes, Ind.). Good Samaritan Hospital is a 232-bed com- munity healthcare facility with more than 1,900 employees. Founded on the Good Samaritan par- able in the Bible, the hospital has been serving southwestern Indiana and southeastern Illinois for more than 100 years.
