14 Boone County Health Center (Albion, Neb.). Boone County Health Center admitted 71 patients and delivered 47 babies in 1950, its inaugural year. Today, physicians and surgeons perform 60,000 procedures at the hospital annually. In the last 10 years, the Health Center has undergone four major infrastructure projects that have transformed it into a progressive acute care facility. e hospital is affiliated with five medical clinics and employs 250 community members. Bridgton (Maine) Hospital. Bridgton Hospital has taken major steps in the last two decades to expand services and improve operations. In 1999, Bridgton became a subsidiary of Central Maine Medical Center to improve access to care among rural patients. e hospital specializes in women's health, providing fertility, breast and cervical cancer, bladder control and menopause services to rural women and families. e National Rural Health Association named Bridgton Hospital a Top 20 Critical Access Hospital for quality performance in 2016. Calumet Medical Center (Chilton, Wis.). Calumet Medical Center is a subsidiary of Affinity Health System, an Appleton, Wis.-based health system owned by St. Louis-based Ascension. Guided by its mission to improve healthcare access in rural communities, CMC recently underwent a large expansion project, beginning in 2006, which included updates to technology and infrastructure, improved patient privacy and enhancements to outpatient services, emergency services and the radiology department. e National Rural Health Association named CMC a Top 20 Critical Access Hospital for patient satisfaction in 2016. CHI St. Alexius Health Carrington (N.D.) Health Center. CHI St. Alexius Health Carrington is a 25-bed hospital that began providing care in 1916 as Carrington Hospital. e hospital includes two rural health centers and has implemented community outreach programs for obesity, childhood services and youth alcohol abuse. In April 2016, Carrington joined Bismarck, N.D.-based CHI St. Alexius Health, a regional system comprised of a tertiary hospital and seven CAHs. e National Rural Health Association named Carrington a Top 20 Critical Access Hospital in 2017. Central Montana Medical Center (Lewistown). is nonprofit, 25- bed hospital is an important part of the economic and social fabric of its community, with the nearest hospital located more than 100 miles from Lewiston. It provides a full range of services, from rehabilitation to intensive care, and earned a Small Health Care Provider grant from HHS in 2017, under which it will receive three years of federal funding for quality improvement initiatives. CMMC was named a Top 20 Critical Access Hospital by the National Rural Health Association in 2017. CHI St. Alexius Health Dickinson (N.D.). Dickinson is the only hospital in a 100-mile radius offering Level IV trauma services, including stroke, cardiac and burn care. e clinical care administered at Dickinson has earned the hospital national recognition. It was a 2013 Top Performer on Key Quality Measures for pneumonia and surgical care, as recognized by e Joint Commission, and it currently holds e Joint Commission's National Quality Approval award. Dickinson continues to expand its services to better serve its community, from obstetrics to pain management. Community HealthCare System, Onaga (Kan.) & St. Mary's. Community HealthCare System is rooted in caring for rural communities. Its senior care initiative, Community HomeHealth, has earned the hospital system national recognition for clinical outcomes, ranging from one of the U.S. News & World Report's Best Nursing Homes in 2016 to HomeCare Elite's Top 500 Home Care Agency 2016. Community HealthCare System's high-quality care goes beyond senior services — it was named a Top 20 Critical Access Hospital 2017 for overall quality by the National Rural Health Association. Delta (Utah) Community Hospital. Salt Lake City-based Intermountain Healthcare opened 18-bed Delta Community Hospital in May 1985 to serve the medical needs of the region. is dedication to community governs the hospital's day-to-day operations. Each year, DCH sponsors a health fair for Delta and the surrounding community, offering health screenings and wellness activities. It also sponsors yearly nutrition and fitness programs for fourth graders at local elementary schools. DCH placed on the Top 100 Critical Access Hospital list by iVantage Health Analytics in 2016. Door County Medical Center (Sturgeon Bay, Wis.). Door County Medical Center has served patients in Wisconsin's Door and Kewaunee counties for 70-plus years. e hospital employs more than 175 physicians who staff a Women's and Children's Center, the Bone and Joint Center of Door County and the Door County Cancer Center. e National Rural Health Association recognized Door County Medical Center as a Top 20 Critical Access Hospital in the U.S. Fairview Hospital (Great Barrington, Mass.). Fairview is a nonprofit, 25-bed member hospital of Pittsfield, Mass.-based Berkshire Health Systems. Fairview Hospital is well-renowned for the high caliber of its clinical care. Most recently, the hospital was recognized among the top 10 percent nationwide for excellence in patient safety by Healthgrades and ranked in the 90th percentile of hospitals nationally for overall care by CareChex in 2017. Floyd Valley Healthcare (Le Mars, Iowa). Floyd Valley Healthcare offers more than the typical rural hospital experience — it also provides quality family medicine clinics, specialty clinics, rehabilitation and speech therapies, home health services, assisted living facilities as well as durable medical equipment production and sales. FVH undertook an expansive renovation project in 2014, adding more than 23,000 square feet to enhance patient services and clinical care. It was named a Top 20 Critical Access Hospital by the National Rural Health Association in 2017. Gunnison Valley (Colo.) Hospital. Situated in a rural, alpine community, GVH is accustomed to dealing with extreme medical circumstances, from cold weather trauma injuries to high-altitude care. e 24-bed critical access hospital and its EMS serve an area nearly twice the size of Delaware. e hospital's commitment to excellence and community has not gone unnoticed. GVH has been named one of iVantage Health Analytics' Top 100 Critical Access Hospitals for six consecutive years, most recently in 2017, and earned distinction from iVantage as a HealthStrong Hospital in 2015. Hammond-Henry Hospital (Geneseo, Ill.). Hammond-Henry Hospital has served northeastern Illinois since 1901. Its commitment to community-based healthcare, clinical excellence and technology is well known. In 2016, the hospital's skilled nursing care earned the Embracing Quality Award for customer satisfaction and achievement from Providigm, as well as a five-star quality rating from CMS for the fih consecutive year. Hospitals & Health Networks named HHH one of HealthCare's Most Wired hospitals in 2016. Hancock County Hospital (Sneedville, Tenn.). Hancock County Hospital, part of Kingsport, Tenn.-based Wellmont Health System, opened in 2005 to provide high-quality healthcare services to the rural Sneedville community. Its services include emergency care, inpatient care, laboratory tests, diagnostic imaging and physical therapy. e
