26 patients with chronic diseases. In January, MemorialCare Regional ACO was one of 21 organizations selected to participate in CMS' Next Gener- ation ACO model for 2016. As of July, MemorialCare Regional is one of 18 ACOs to have successfully remained in the program. Mercy ACO (Des Moines, Iowa). Mercy ACO was formed in 2012 by Mercy Medical Center in Des Moines and Wellmark Blue Cross and Blue Shield of Iowa. e ACO has participated in the Medicare Shared Savings Program for the past four years, during which it has been recognized for its improvements in patient quality measures. Mercy ranked among the top 10 MSSP ACOs for quality in 2014. Mercy Health Select (Cincinnati). CMS selected Mercy Health Select for its Medicare Shared Savings Program in 2014, shortly aer it launched a commercial contract with Humana. Mercy Health Select was among 10 ACOs in the program to generate the largest amount of shared savings in 2014 of $6.52 million. Michigan Pioneer (Southfield, Mich.). Affiliated with Detroit Medical Center, Michigan Pioneer ACO is one of nine remaining ACOs partici- pating in CMS' Pioneer program. e ACO generated the fourth great- est sum in shared savings, $9.82 million, in the Pioneer ACO program in 2014. Michigan Pioneer was the most successful Pioneer ACO for benchmark savings improvement year-over-year. e ACO administers care to more than 20,000 Medicare beneficiaries. Millennium ACO (Fort Myers, Fla.). Millennium Physician Group and ProCare Med formed an ACO in 2013 to treat Medicare beneficiaries across southwest Florida. It generated the sixth largest sum in shared savings, $7.98 million, in the Medicare Shared Savings Program in 2014. Millennium Physician Group has since joined Florida Blue's ACO, which launched in 2015. MissionPoint Health Partners (Nashville). Founded by Saint omas Health in Nashville, MissionPoint Health Partners joined CMS' Medi- care Shared Savings Program in 2012. e ACO became a direct subsid- iary of St. Louis-based Ascension Health and works closely with St. Vin- cent Health in Indianapolis. By the end of 2016, MissionPoint expects to serve more than 250,000 patients across the central United States. Monarch HealthCare ACO (Irvine, Calif.). Monarch HealthCare ACO serves approximately 176,000 Orange County residents. Under the Pio- neer ACO Model, Monarch HealthCare saved 5.4 percent of forecasted medical costs in 2013, making it the second highest performer in the Pioneer program that year. Monarch HealthCare ACO was among the 20 percent of Medicare ACOs that qualified to share savings in 2014, generating $5.99 million. Montefiore ACO (Bronx, N.Y.). Despite serving one of the most dense- ly populated, low-income communities in the United States, Montefiore ACO is one of the more successful and progressive healthcare providers in the Pioneer ACO program. Montefiore generated 3.6 percent in gross savings and generated $8.43 million in shared savings in 2014. Monte- fiore ACO has participated in CMS' Pioneer ACO program since 2011. Mosaic Life Care (St. Joseph, Mo.). e Mosaic Life Care ACO launched in 2012 with a total of 29,000 patients. In the second year of its three-year contract, Mosaic achieved the second highest quality score of all MSSP ACOs, earning a 95.1 percent rating overall. e ACO generated $2.1 million in shared savings in 2014. Mount Sinai Health System (New York City). Mount Sinai Health Part- ners, a clinically integrated network including providers and Mount Si- nai Health System, partnered with health insurer Aetna to form an ACO in February 2016. e two groups signed a three-year contract that en- ables 1.1 million Aetna policyholders in New York to receive care from 3,100 employed and affiliated Mount Sinai physicians. Currently, Mount Sinai has ACO arrangements with CMS, Healthfirst and Empire. National Rural ACO (Nevada City, Calif.). NRACO became a MSSP ACO in 2014 and is comprised of nine rural hospital members, some of them critical access facilities, spread across different regions of the country — California, Indiana and Michigan. e National Rural ACO model enables rural providers to participate in CMS programs that don't have the resources to do it on their own. NRACO leaders created the NRACO Services Corporation in 2013, which organized more than 159 rural health systems into 23 MSSP ACOs in 2016. NH Accountable Care Partners (Concord, N.H.). Dover, N.H.-based Wentworth-Douglas Hospital and Southern New Hampshire Health System in Nashua have joined Concord (N.H.) Hospital and Manchester, N.H.-based Elliot Hospital in an accountable care organization in 2014. NH Accountable Care Partners was among the top 10 MSSP ACOs for quality in 2014, earning a 93.6 percent rating overall. It serves more than 30,000 Medicare beneficiaries. Northwell Health (Great Neck, N.Y.). Building on a two decade rela- tionship, Great Neck, N.Y.-based Northwell Health and New York-based EmblemHealth created a joint venture in 2016. CMS selected Northwell Health as one of 100 new MSSP ACOs to join the program this year. Northwestern Medicine (Chicago). Northwestern Medicine, a collabo- ration between four-hospital Northwestern Memorial HealthCare and Northwestern University's Feinberg School of Medicine, launched an ACO with Cigna in 2015. e ACO serves more than 5,600 Cigna policy- holders in the Chicago area. It is the fourth ACO Cigna has co-launched in the area. Oakwood ACO (Dearborn, Mich.) Oakwood ACO started with a bang, generating $17 million in Medicare savings during its first 18 months in op- eration, from July 2012 to December 2013. e ACO produced $8.15 million in shared shavings in 2014, placing it among the top 10 MSSP ACOs for savings generated. is year, Oakwood ACO teamed up with eight-hospital Beaumont Health in Royal Oak, Mich., and United Physicians in Bingham Farms, Mich., to create a clinically integrated care network. OneCare Vermont (Colchester, Vt.). Dartmouth-Hitchcock in Leba- non, N.H., and the University of Vermont Medical Center in Burlington launched OneCare Vermont in 2013 and is one of the country's largest ACOs by physician network size. OneCare Vermont was chosen as Ver- mont's exclusive partner in a Medicaid initiative that aims to mirror the Next Generation ACO model. OneCare is an MSSP ACO. Optum ACO (Phoenix). Formed in 2014, Optum ACO is the collab- oration between Phoenix-based Banner Health Network and United- Healthcare. Banner coordinates and manages care for more than 50,000 UnitedHealthcare policyholders. is year, Optum ACO was selected to participate in CMS' Next Generation ACO initiative. Orlando (Fla.) Health. Orlando Health ACO is a collaboration between Orlando Health, which includes Orlando Health Physician Partners, and health insurer Florida Blue. CMS selected Orlando Health as a MSSP ACO in 2013. is year, CMS selected Orlando Health ACO to partic- ipate in a series of webinars featuring ACO success stories based on re- sults from the first two years of performance. OSF Healthcare System (Peoria, Ill.). OSF Healthcare System created an ACO under the Pioneer model in 2011 and landed its first commercial ac- countable care agreement with Blue Cross Blue Shield of Illinois for 40,000 beneficiaries in 2013. e ACO earned the fih highest quality score among Pioneer ACOs in 2014, achieving a score of 90.3 percent. OSF Healthcare System ACO transitioned into a Next Generation ACO in 2016. Palm Beach ACO (Palm Springs, Fla.). Palm Beach ACO began the Medicare Shared Savings Program in July 2012. In fiscal year 2014, the most recent year for which data is available, it earned $14.5 million, mak- ing Palm Beach ACO the No. 2 MSSP ACO for shared savings to Memo-
