As organizations focus on patient-centeredness and integrated care, The Joint Commission
brings to the forefront a new certifi cation program focusing on care coordination – how the patient
progresses from inpatient to outpatient settings and how that care is delivered.
Integrated Care Certifi cation evaluates how well an organization links information sharing,
transitions of care, hand-off communications and other key activities. The voluntary certifi cation
sets the foundation for successfully using data to improve outcomes and ensures that necessary
tools and processes are in place.
With a focus on leadership, clinical integration, patient and family engagement, and so much more,
this new certifi cation is the starting point for improving patient outcomes and, more importantly,
making health care seamless for both providers and patients.
