For more than two decades, Dr. Cohen has served at the helm of ISMP, a non- profit healthcare organization that specializes in understanding the causes of medication errors and providing error-reduction strategies to the health- care community, policy makers and public. Previ- ously, Dr. Cohen served as vice chair of the Patient Safety Advisory Group for the Joint Commission, which created the National Patient Safety Goals. He is editor of the textbook "Medication Errors" and serves as co-editor of the ISMP Medication Safety Alert! publications that reach more than 2 mil- lion health professionals and consumers. In 2014, Dr. Cohen was made an honorary member of the American Society of Health-System Pharmacists. Patrick Conway, MD. CMS Deputy Adminis- trator for Innovation and Quality and CMO of CMS. As deputy administrator and CMO of CMS, Dr. Conway leads the Center for Clinical Standards and Quality — the center responsible for all qual- ity measures, quality improvement programs and clinical standards in all 50 states — and the Cen- ter for Medicare and Medicaid Innovation. Com- bined, CCSQ and CMMI's budgets exceed $12 billion. Previous positions Dr. Conway has held include director of hospital medicine at Cincinnati Children's Hospital, assistant professor at Cincin- nati Children's Hospital Medical Center and CMO of HHS, to name a few. In February, Dr. Conway was a speaker at The Advisory Board Company's inaugural Future of Health Care Summit, where he discussed how patient safety improvements have led to $12 billion in savings from 2010 to 2014. Katrina Crist. CEO of the Association for Profes- sionals in Infection Control and Prevention. As CEO of APIC, Ms. Crist oversees the collection and analysis of data to track trends in infection prevention and control programs and the role of the infection preventionist, public policy initiatives and delivery of education for healthcare personnel and the public about infectious diseases. Ms. Crist was also key in helping develop the APIC Strategic Plan 2020, "toward healthcare without infection." Prior to joining APIC, Ms. Crist served as executive director with the American Society of Transplant Surgeons, which represents the interests of organ and cell transplant surgeons across the country. Jacqueline Daley. Corporate Director of Infection Prevention and Control at LifeBridge Health-Si- nai Hospital (Baltimore). Ms. Daley became di- rector of infection prevention and control at Sinai Hospital in Baltimore in 2014. She has co-authored several publications on reducing surgical site in- fections. She also implemented a "zero tolerance" policy at Sinai. Since the policy's implementation, the SSI has decreased by 70 percent at the hospital. Krishna Das, MD. Chief Quality Officer for Cook County Health & Hospitals System (Chicago). As chief quality officer, Dr. Das leads quality improve- ment and patient safety initiatives for the third larg- est public hospital system in the nation. CCHHS is comprised of two acute-care hospitals, 16 commu- nity-based health clinics, a correctional health fa- cility and the Cook County Department of Public Health.
