38 Patient Satisfaction and Safety A s of April 16, potential patients can see star ratings of hospitals on CMS' Hospi- tal Compare website. The star ratings are based on HCAHPS scores. CMS added the star ratings to "make it easier for consumers to use the information on the Compare Web sites and spotlight excellence in healthcare quality," according to the HCAHPS website. Each hospital will have 12 star ratings — one for each of the 11 publicly reported HCAHPS measures, and one as the HCAHPS Summary Star Rating. The Summary Star Rating combines the 11 HCAHPS star ratings into one comprehensive metric. "These star ratings also encourage hospitals and clinicians to strive to continuously im- prove the patient experience and quality of care delivered to all patients," said Patrick Conway, MD, acting principle deputy administrator for CMS and deputy administrator for innovation and quality. As of April 16, 3,553 hospitals have a Summary Star Rating ranging from one (the worst) to five (the best) stars. Hospitals must have at least 100 completed surveys in a 12-month period to re- ceive a star rating. The following is a breakdown of how many hos- pitals received each star rating. • 1 star: 101 hospitals (3 percent) • 2 stars: 582 hospitals (15 percent) • 3 stars: 1,414 hospitals (40 percent) • 4 stars: 1,205 hospitals (34 percent) • 5 stars: 251 hospitals (7 percent) The HCAHPS star ratings will be updated each quarter, according to CMS. n Star Ratings Go Live on Hospital Compare: How Many Hospitals Got 5 Stars? By Heather Punke 7 Procedures Where Patient Safety Events Occur Most Often By Heather Punke T hree-quarters of hospital-reported patient safety events were concentrated within four patient safety indicators, according to analysis from Healthgrades, and those four events occurred most often in seven specific procedures or conditions. Healthgrades examined 14 patient safety indicators defined by the Agency for Healthcare Research and Quality and inpatient data from the Medicare Provider Analysis and Review database and software from AHRQ. The findings were published in a report, "Asking the Right Questions to Improve Patient Experience and Safety." Together, the following four PSIs made up 75 percent of all patient safety events: • Accidental cut, puncture, perforation or hemorrhage during medical care • Collapsed lung due to a procedure or surgery around the chest • Catheter-related bloodstream infections acquired at the hospital • Pressure sores or bed sores acquired in the hospital According to Healthgrades, those four patient safety events occur most often in the following seven procedures or conditions: • Bowel obstruction surgery • Coronary artery bypass graft surgery • Colorectal surgery • Hip fracture repair • Sepsis • Small intestine surgery • Pacemaker implantation "Hospitals should focus their evaluation and preventive measures within these cohorts, streamlin- ing their efforts for maximum results," said Evan Marks, chief strategy officer with Healthgrades. n Which States Have the Most CMS 5-Star Hospitals? By Heather Punke J ust 251 hospitals earned the highest possible star rating as part of CMS' Hospital Com- pare's new rating system, and some states have many more five-star hospitals than others. Texas has the most five-star hospitals, clocking in at 31, according to numbers provided by Kaiser Health News. The following are the eight states that have 10 or more five-star hospitals within their borders. 1. Texas — 31 2. Wisconsin — 25 3. Louisiana — 17 4. Ohio — 15 5. Illinois — 13 6. Minnesota — 12 7.
