The IPA includes more than 200 primary care physicians across five counties. Robert Wood Johnson Partners (New Bruns- wick, N.J.). Robert Wood Johnson University Hospital and Health System in New Brunswick collaborated with Piscataway Township, N.J.- based Rutgers University for the creation of an ACO called the Robert Wood Johnson Partners, which began participating in the Medicare Shared Savings Program in late 2013. The organization involves academic researchers working with phy- sicians to bring the strengths of a major research university to its patients. Rochester (N.Y.) General Health System ACO. Rochester General Health System ACO was formed in July 2013 and joined the Medicare Shared Savings Program in 2014. It is a partner- ship between Rochester General Health System and the Greater Rochester Independent Practice Association, which includes more than 960 physi- cians. Additionally, the ACO entered into a five- year ACO agreement with Excellus BlueCross BlueShield in January 2013. That agreement cov- ers about 110,000 patients. Scott & White Healthcare Walgreens Well Net- work (Temple, Texas). Scott & White Healthcare Walgreens Well Network joined the Medicare Shared Savings Program in January 2013. The ACO is a partnership between drug retailing chain Walgreens and Scott & White Healthcare, a physician-owned multispecialty medical group practice. Scott & White is now part of Baylor Scott & White Health. Scottsdale (Ariz.) Health Partners. Scottsdale Health Partners, a clinically integrated network, is a joint venture between Scottsdale Healthcare and Scottsdale Physician Organization, which includes more than 500 community physicians. SHP partnered with Cigna for a collaborative ac- countable care initiative in 2013 and became a Medicare Shared Savings Program ACO in 2014. Sharp HealthCare ACO (San Diego). Sharp HealthCare was selected as a Medicare Pioneer ACO in 2011. The ACO, which includes seven hospitals and two medical groups, serves more than 32,000 Medicare beneficiaries. In its first performance year in 2012, Sharp's ACO did not achieve enough savings to share in them with CMS. SSM ACO (St. Louis). The SSM ACO, formed by SSM Health Care, joined the Medicare Shared Savings Program in 2014. SSM Health Care also joined Missouri Anthem Blue Cross and Blue Shield's patient-centered care program this year, creating an accountable care model to help An- them members manage their chronic conditions and improve their health. St. John Providence Health System (War- ren, Mich.). St. John Providence Health System formed St. John Providence Partners in Care in 2011 as a 50/50 partnership with The Physician Alliance, a physician organization with more than 2,300 physicians, to pursue ACO contracts. The ACO joined the Medicare Shared Savings Pro- gram in 2013. St. Joseph Health System (Orange, Calif.). In 2011, St. Joseph Health partnered with Blue Shield of California for an ACO covering about 30,000 Blue Shield HMO members in Orange County. The ACO includes three hospitals, the St. Joseph Home Health Ministry, three affiliated physician networks and three medical groups. St. Luke's Clinic Coordinated Care (Boise, Ida- ho). St. Luke's Clinic Coordinated Care joined the Medicare Shared Savings Program in January 2013. It was the first Idaho-based ACO accepted into the program. The ACO is a joint venture be- tween the St. Luke's Health System's hospitals and physicians. Steward Promise (Boston). Steward Promise is the one of the remaining 23 Medicare Pioneer ACOs. It's the ACO of Steward Health Care Net- work, which is the second largest physician net- work in Massachusetts and affiliated with Bos- ton-based Steward Health Care System. SHCN includes 2,800 providers who treat more than one million patients annually. Steward Promise was able to deliver savings in its first reporting year in 2012. Tenet Healthcare (Dallas). Tenet Healthcare has several ACO agreements in different states. In 2012, the for-profit system signed a national, multi-year accountable care agreement with Cig- na for healthcare services owned and operated by Tenet subsidiaries. In 2013, Tenet announced a partnership with Blue Cross and Blue Shield of Texas for a statewide ACO that will be available to BCBSTX PPO patients starting Jan. 1, 2015. This year, Tenet struck a partnership with Florida Blue for an ACO covering about 10,000 Florida Blue members. Tenet also has an ACO agreement with Blue Shield of California. Texas Health Resources (Arlington). Texas Health Resources started its ACO journey with North Texas Specialty Physicians for a Pioneer ACO called Plus; however, it became one of the nine ACOs to exit the program in 2013. The sys- tem and Texas Health Physicians Group are still involved in accountable care efforts with com- mercial payers. They partnered with Cigna for a collaborative accountable care initiative in 2014, and Texas Health Resources also has ACO agree- ments with Aetna and Blue Cross Blue Shield of Texas. Triad HealthCare Network (Greensboro, N.C.). The Triad HealthCare Network is a partnership between community physicians and Greensboro- based Cone Health System. THN became a mem- ber of the Medicare Shared Savings Program in July 2012. The organization is comprised of 850 physicians, 360 of which are employed by Cone Health. Trinity Pioneer ACO (Fort Dodge, Iowa). This Medicare Pioneer ACO is anchored by Trinity Regional Medical Center, UnityPoint Clinic-Fort Dodge Region, Berryhill Center and UnityPoint at Home-Fort Dodge Region. Trinity Pioneer ACO is the most rural of the Pioneer ACOs with one of the smallest population bases. It did not generate enough savings to share in them with Medicare in its first performance year in 2012. UCLA Health (Los Angeles). UCLA Health is currently participating in two ACO models. It joined the Medicare Shared Savings Program in January 2013 in an ACO that includes the sys- tem's hospitals and the UCLA Medical Group. Then, in October 2013, UCLA Medical Group joined Anthem Blue Cross' ACO network to care for Anthem PPO members who have two or more chronic conditions. UnityPoint Health Partners (West Des Moines, Iowa). UnityPoint Health Partners is a Medicare Shared Savings Program participant. The ACO includes hospitals, other facilities and provider groups from around Iowa, Illinois and Wisconsin. This year, UnityPoint Health Partners also joined with UnitedHealthcare for an ACO covering more than 36,000 UnitedHealthcare members. University Hospitals (Cleveland). University Hospitals boasts three main ACOs: a pediatric ACO, an employee ACO and a Medicare Shared Savings Program ACO. UH established its em- ployee ACO in 2010 for roughly 25,000 employ- ees and dependents. It joined the MSSP in 2012, and that ACO includes six medical centers, a re- habilitation hospital and 20 outpatient centers. The pediatric ACO, UH Rainbow Care Connec- tion, helps manage the health of 200,000 children in the area, one-third of whom are on Medicaid. The Physician Organization of Michigan ACO (Ann Arbor). This Medicare Shared Savings ACO added more than 2,900 physicians to its network this year, bringing its provider total to 5,700. It covers about 120,000 Medicare beneficiaries. POM ACO started as a joint venture between University of Michigan Health System and eight physician groups. It took on members of the Uni- versity of Michigan Pioneer ACO, which dropped out of CMS' Pioneer ACO program in 2013, as well as providers from Midland, Mich.-based MidMichigan Health, the nonprofit health system affiliated with U-M Health System. UW Health ACO (Madison, Wis.). UW Health ACO is a partnership between University of Wis- consin Health physicians, clinics, hospitals and medical school. It joined the Medicare Shared Savings Program in January 2013. Jonathan Jaf- fery, MD, serves as president and CEO of the ACO. VirtuaCare ACO (Marlton, N.J.). VirtuaCare ACO is the Medicare Shared Savings ACO of Vir- tua, a four-hospital system in southern New Jer- sey. VirtuaCare joined the MSSP program in Jan- uary 2013. VirtuaCare ACO was also one of seven ACOs to enter a collaborative relationship with SilverScript Insurance Company, a subsidiary of CVS Caremark, in June. The alliance is intended to upgrade pharmacy care for patients with Medi- care Part D coverage. n
