Bellin-ThedaCare Healthcare Part- ners is the ACO between Green Bay-based Bellin Health, Neenah, Wis.-based ThedaCare and a network of nearly 700 physicians. It was named one of CMS' first 32 Pioneer ACOs in 2011 and achieved a 4.6 percent reduction in the total cost of care for 20,000 Medicare beneficiaries from 2011-2012. Beth Israel Deaconess Care Organization (Westwood, Mass.). BIDCO has a tertiary affili- ation with Boston-based Beth Israel Deaconess Medical Center. The ACO includes more than 2,100 primary care physicians and specialists along with numerous community hospitals, in- cluding Beth Israel Deaconess Hospital-Milton, Beth Israel Deaconess Hospital-Needham, Beth Israel Deaconess Hospital-Plymouth and the Cambridge Health Alliance. BIDCO was named one of CMS' first 32 Pioneer ACOs in late 2011. Billings (Mont.) Clinic. Billings Clinic was se- lected as a Medicare Shared Savings Program ACO in January 2013. That same month, Billings Clinic was one of the first four organizations in the country to receive ACO accreditation from the National Committee for Quality Assurance. BJC HealthCare ACO (St. Louis). The first ACO in the St. Louis area, BJC HealthCare ACO was se- lected by CMS for participation in the Medicare Shared Savings Program in July 2012. The ACO is affiliated with 12-hospital BJC Healthcare, in- cludes more than 100 healthcare delivery settings and delivers care through an integrated network of providers. Sandra Van Trease serves as presi- dent of the ACO. Brown & Toland Medical Group (San Francis- co). Brown & Toland Physicians, an independent physician organization, was selected by CMS as a Pioneer ACO in late 2011. The ACO includes ap- proximately 190 physicians in San Francisco and the East Bay who care for more than 18,000 Medi- care beneficiaries. Brown & Toland Physicians helped generate savings for Medicare in its first performance year as a Pioneer. Bon Secours Health System (Marriottsville, Md.). Bon Secours Health System's Good Help ACO was approved by CMS in December 2012 to partici- pate in the Medicare Shared Savings Program. The ACO covers approximately 57,000 Medicare lives across five states. In June 2013, Good Help ACO announced a collaboration with Aetna subsidiary Healthagen to coordinate healthcare for Medicare beneficiaries whose primary physician participates in the Good Help ACO. Cape Cod Health Network ACO (Hyannis, Mass.). Cape Cod Healthcare and Cape Cod Pre- ferred Physicians, a group of approximately 270 physicians, collaborate for their ACO, called Cape Cod Health Network. CMS selected the ACO as a participant in the Medicare Shared Savings Pro- gram in January 2013. Carilion Clinic (Roanoke, Va.). Carilion Clinic formed its ACO through a partnership with Aetna in 2011. The goal of the ACO is to personalize the care needs of patients, including Virginia's Med- icaid beneficiaries, and to cut healthcare costs. Carilion Clinic also has an ACO that participates in the Medicare Shared Savings Program, called Doctors Connected. Carolinas HealthCare System (Charlotte, N.C.). Carolinas HealthCare System has more than 791 care locations throughout the Carolinas. The sys- tem participated in the Partnership for Care Trans- formation Readiness Collaborative with Premier, which was launched in 2013 to help organizations decide if they should pursue an ACO model. In 2014, the system announced a collaboration with Aetna to support patient-centered medical homes and other accountable care initiatives. CaroMont Health (Gastonia, N.C.). CaroMont's ACO began participating in CMS' Medicare Shared Savings Program in January 2014. The ACO is made up of CaroMont's acute-care hos- pitals, medical group, hospice, surgical center and women's center. CaroMont also entered a collab- orative accountable care agreement with Cigna in July 2013, which covers more than 3,000 people. Cedars-Sinai Accountable Care (Beverly Hills, Ca- lif.). Cedars-Sinai Accountable Care, anchored by 958-bed Cedars-Sinai Medical Center, was selected to participate in the Medicare Shared Savings Pro- gram in January 2013. Cedars-Sinai Medical Care Foundation providers also participate in the ACO. Children's Hospital of Philadelphia. The Chil- dren's Hospital of Philadelphia was the first chil- dren's hospital to earn National Committee for Quality Assurance accreditation as an ACO. The ACO accreditation is based on infrastructure that supports quality improvement, patient-centered care and care coordination. Cleveland Clinic Florida (Weston). Cleveland Clinic Florida is a nonprofit academic medical cen- ter and extension of Cleveland Clinic in Ohio, with more than 215 physicians. In February 2013, the hospital and Florida Blue signed a letter of intent to form an ACO. The ACO has a value-based compen- sation structure, rewarding physicians for transpar- ency, care coordination and lack of redundancies. Dartmouth-Hitchcock (Lebanon, N.H.). Dart- mouth-Hitchcock's ACO is one of Medicare's original 32 Pioneers and covers more than 17,000 patients. In its first performance year in 2012, the health system-anchored ACO saved enough money to receive about $1 million in shared savings from Medicare. Dartmouth-Hitchcock also participated in the Medicare Physician Group Practice Demon- stration, which preceded the Pioneer model, and has similar value-based arrangements with pri- vate payers. Additionally, Dartmouth-Hitchcock co-created Medicare ACO OneCare Vermont with Burlington, Vt.-based Fletcher Allen. Dean Clinic and St. Mary's Hospital ACO (Madi- son, Wis.). St. Mary's Hospital in Madison and its affiliated physician group formed a joint venture to launch an ACO, which was selected to participate in the Medicare Shared Savings Program in July 2012. The ACO serves patients in south central Wisconsin. Dignity Health (San Francisco). Along with San Ramon, Calif.-based Hill Physicians and Blue Shield of California, Dignity Health, then doing busi- ness as Catholic Healthcare West, launched a pilot program in 2010 to manage the care of more than 40,000 members of the California Public Employ- ees' Retirement System. Based on the pilot's success, the system finalized an accountable care agreement with the City and County of San Francisco and Blue Shield of California, in 2011. In 2014, Blue Shield, Hill Physicians Medical Group and Dignity Health announced a three-year accountable care initiative covering approximately 14,600 Blue Shield HMO members in San Joaquin County. Duke Connected Care (Durham, N.C.). Duke Connected Care, the ACO affiliated with Durham, N.C.-based Duke University Health System, is a physician-led group that was selected to participate in the Medicare Shared Savings Program in 2014. Emory Healthcare (Atlanta). Emory Healthcare, the largest health system in Georgia, signed an ACO agreement with Blue Cross and Blue Shield of Georgia in January 2014. Emory's Healthcare Network includes six hospitals. The partnership between the organizations is designed to improve quality of care and reduce medical costs by in- creasing collaboration and efficiency. Essentia Health (Duluth, Minn.). Essentia Health was chosen in July 2012 to participate in the Medicare Shared Savings Program as an ACO with more than 33,000 covered lives. In fall 2013, Essentia Heal was one of two organizations in the country to receive level 3 ACO designation from the National Committee for Quality Assurance. Fairview Health Services (Minneapolis). Fair- view Health Services was one of the 32 Medicare Pioneer ACOs, named in 2012. In 2013, Fairview Health Services and North Memorial Health Care in Robbinsdale, Minn., partnered with health in- surer Medica to create Fairview and North Memo- rial Vantage with Medica, Minnesota's largest ACO. Franciscan Alliance (Mishawaka, Ind.). Francis- can Alliance, a 13-hospital system, has four Medi- care ACOs: one in the Pioneer program and three in the Medicare Shared Savings Program. Each is comprised of various Franciscan Alliance hospi- tals, affiliated physician groups and surgery cen- ters. The Pioneer ACO covers more than 20,000 lives. The MSSP ACOs are the Franciscan AHN ACO, Franciscan Select Health Network ACO and Franciscan Union ACO. The system also has com- mercial ACOs with Cigna and Aetna. Genesys Physician Hospital Organization (Flint, Mich.). The Genesys Physician Hospital Organiza- tion, a collaboration between Genesys Health Sys- tem and Genesys Physicians Group Practice, was created in 1994. It includes about 160 primary care physicians and 400 specialists. CMS selected Gene- sys Physician Hospital Organizations as one of the original 32 Pioneer ACOs in late 2011. Hackensack (N.J.) Alliance ACO. Hackensack University Medical Center, a nonprofit teaching hospital, anchors the Hackensack Alliance ACO. Hackensack Alliance was established to partici-
