General Comprehensive Unit-Based Safety Program Toolkit: The Agency for Healthcare Research and Quality offers the General CUSP Toolkit for comprehensive unit-based safety, an instructional resource for unit leaders to teach team members about understanding safety and incorporating safety considerations into daily operations. The six-module tool includes slides, videos and facilitator notes. The following is a sampling of tools recently published on Becker's Healthcare websites and available for immediate download. AHRQ's Preventing Falls Toolkit: The Agency for Healthcare Research and Quality offers a free preventing falls toolkit to help hospitals implement a fall prevention program. The toolkit was developed by AHRQ, the RAND Corp., ECRI Institute and Boston University and includes resources for stakeholders and care teams. APIC's Guide to C. diff Infection Prevention: The Association for Professionals in Infection Control and Epidemiology offers "Guide to Preventing Clostridium Difficile Infections," an update of a 2008 APIC guide. The revised guide document details interventions to prevent C. diff, including surveillance, hand hygiene, contact/isolation precautions, environmental infection prevention and antimicrobial stewardship AORN's Surgical Checklist: The Association of periOperative Registered Nurses offers a free surgical checklist to ensure patient safety and help facilities meet The Joint Commission's Universal Protocol requirements. The checklist was created in 2010 by representatives from AORN and The Joint Commission and contributors to the World Health Organization Surgical Safety Checklist. CDC Checklist for CLABSI Prevention: The Centers for Disease Control and Prevention offers a free checklist, based on 2011 CDC guidelines for the prevention of intravascular catheterassociated bloodstream infections, for CLABSI prevention. The checklist includes tools for both clinicians and healthcare organizations. FAQs About CAUTIs Poster: The CDC provides a free, downloadable poster identifying frequently asked questions about catheter-associated urinary tract infections. The poster addresses the basics of urinary catheters, CAUTIs and CAUTI prevention and includes tips for patients with catheters upon discharge. Glove Use Information Brochure: The World Health Organization offers a four-page brochure on glove use. The brochure covers a number of topics, including the technique for donning and removing non-sterile examination gloves, the need for hand hygiene, inappropriate glove use, a decision-making aid on when to gloves and recommendations on general glove use. Injection Safety Brochure: The Safe Injection Practices Coalition provides the Injection Safety Brochure, targeted toward healthcare providers, to improve injection practices in healthcare settings. The brochure contains a summary of material on safe injection practices, injection safety tips from the CDC and an infographic on injection-transmitted disease. The Joint Commission Guide to Reducing SSIs: "The Joint Commission's Implementation Guide for NPSG.07.05.01 on Surgical Site Infections: The SSI Change Project," is based on The Joint Commission's SSI Change Project, an initiative to reduce SSIs. The Joint Commission's goal, stated in NPSG 07.05.01, is to implement evidence-based practices for preventing SSIs. It is based on 17 hospitals experiences in phase II of the project. Clinical Teach-Back Cards: The TMF Health Quality Institute offers free clinical teach-back cards to help healthcare providers explain medical information to patients. Kaiser Permanente's Infection Control "Plus" Measures: Oakland, Calif.-based Kaiser Permanente offers the 2013 Infection Prevention and Control "Plus" Measures Toolkit to serve as a resource for infection preventionists assessing or expanding infection prevention programs. Developed in 2013 by Sue Barnes, RN, leader of the national infection prevention and control program at Kaiser Permanente, the kit includes infection prevention practices and supporting evidence. The clinical teach-back cards, developed in 2011, explain the concept of teach-back, which is to ask patients open-ended questions to ensure they Outpatient Facility Infection Control Assessment: The Pennsylvania Patient Safety Authority offers a free infection control assessment tool for ambulatory surgery centers. The tool assesses ambulatory surgery facilities' compliance in five areas discussed in the report: surveillance techniques, sterilization and high-level disinfection, safe injections and point-of-care devices, standardized educational programs and environmental infection control and can help leaders identify gaps in infection control practices and develop plans to close the gaps. Safer Sign-Out Form For Patient Handoffs: The Emergency Medicine Patient Safety Foundation offers the Safer Sign Out form to improve the safety of patient handoffs at the end of a shift. The protocol, originally developed by the Safety Leadership Group of Emergency Medicine Associates in Germantown, Md., involves five key components: record, review, round together, relay to the team and receive feedback. Serious Event Response Toolkit: The Institute for Healthcare Improvement offers a free, downloadable toolkit to help healthcare organizations prepare for a response to a serious adverse event. The toolkit includes a process flow chart for communicating serious adverse events, checklists for mitigating the harm after an adverse event and a guideline for debriefing after an adverse event. TeamSTEPPS Training Module for Primary Care: The Agency for Healthcare Research and Quality offers a free TeamSTEPPS training module for primary care teams. TeamSTEPPS is a system designed to improve patient outcomes through enhanced communication and teamwork among healthcare professionals. The primary care training module includes an instructor guide PowerPoint presentation, classroom slides and instructional videos. Universal ICU Decolonization for MRSA Reduction: The Agency for Healthcare Research and Quality and the Centers for Disease Control and Prevention offer instructions for implementing universal decolonization in adult intensive care units to eliminate methicillin-resistant Staphylococcus aureus.
