During the 2017 performance year, BJC HealthCare ACO reported nearly $9 million in shared savings and distributed 67 percent of the savings to participants. e ACO is affiliated with St. Louis-based BJC HealthCare and includes 13 of the system's hospitals. Boston Accountable Care Organization. Boston Accountable Care Or- ganization includes primary care physicians from several Boston area groups, including Boston University Affiliated Physicians. It is a man- aged care organization under MassHealth, which provides administrative functions for managing the network. It is affiliated with Boston Medi- cal Center Health System and aims to innovate in high quality, low cost care. e ACO has 15 participants and covers about 110,000 lives. It also launched a complex care management program that tripled assessment completion rate within 72 hours and reduced inpatient admissions by 29 percent during the three-to-six-month period aer enrollment. Broward Guardian (Hollywood, Fla.). Broward Guardian is a primary care physician organization aligned with Hollywood, Fla.-based Memo- rial Health System. It is a Track 3 MSSP ACO that launched in January 2014. It began its second reporting period in January 2018. During the 2017 performance year, the ACO reported $3.7 million shared savings and distributed 65 percent to ACO participants. Capital Health Accountable Care Organization (Trenton, N.J.). Capi- tal Health Accountable Care Organization serves patients in New Jersey and Pennsylvania. e Track 1 MSSP ACO launched in January 2015 and began its second agreement period in January 2018. During the 2017 performance year, the ACO reported $1.1 million in shared savings and distributed 35 percent to participants, including Pennington, N.J.-based Capital Health System. CaroMont ACO (Gastonia, N.C.). CaroMont ACO launched in Janu- ary 2014 in the Medicare Shared Savings Program. e ACO's 600-plus providers include CaroMont Medical Group, Gaston Memorial Hospital and Hospice of Gaston County, all in Gastonia, N.C. e organization reported $5.1 million in shared savings for the 2016 performance year. It entered a second agreement period for Track 1 in 2017. Centra Alliance (Lynchburg, Va.). Centra Alliance has 14 participants, including Centra Health, a health system that serves more than 500,000 people in central and southern Virginia. e health system has about 400 employed care providers and physicians. e physician-led ACO in- cludes both employed and independent physicians who aim to improve the quality of care in the Lynchburg, Va., area. Carolinas HealthCare System ACO (Charlotte, N.C.). Carolinas Health- Care System ACO has 68 participants, including several hospitals, medical groups and urgent care centers. e ACO is affiliated with Charlotte-based Atrium Health and became a Track 1 MSSP ACO in January 2018. Atrium Health includes more than 40 hospitals and 900 care locations. CHESS Value (High Point, N.C.). CHESS is a Track 1 MSSP ACO that launched in January 2017, serving patients in North Carolina and Vir- ginia. High Point-based Cornerstone Health Enablement Strategic Solu- tions, a physician-led company owned by North Carolina Baptist Hospi- tal in Winston-Salem, N.C., Cornerstone Health Care in High Point, and Laboratory Corporation of America in Burlington, N.C., administers the ACO. Two hospitals and several physician groups are in the ACO. Christus Health Quality Care Alliance (Irving, Texas). Christus Health Quality Care Alliance is a Track 1 MSSP ACO serving patients in Texas, Louisiana and New Mexico. It launched in January 2016 and is in its first agreement period. e ACO is affiliated with Christus Southeast Texas Health System, a nonprofit system with three inpatient hospitals and 27 other points of care. Cleveland Clinic Medicare ACO (Independence, Ohio). Cleveland Clinic Medicare ACO covers members in Ohio and Florida, where Cleve- land Clinic has major campuses. e Track 1+ MSSP ACO launched in 2015 and began its second agreement period in January 2018. During the 2016 performance year, Cleveland Clinic Medicare ACO reported nearly $20 million in shared savings and distributed 90 percent to participants. e ACO is affiliated with Cleveland Clinic, an 18-hospital health system with more than 210 outpatient locations. Coastal Plains Network (Greenville, N.C.). Launched in January 2015, Coastal Plains Network is a Track 1 MSSP ACO that serves patients in North Carolina. It began its second agreement in 2018. During the 2017 performance year, the ACO reported nearly $3.9 million in shared sav- ings and distributed 60 percent to participants, which include Greenville, N.C.-based Vidant Medical Group. ColigoCare (Paramus, N.J.). Although based in New Jersey, ColigoCare serves patients in New York as well. e Track 1 MSSP ACO launched in January 2018 and includes both primary care and specialty physicians. e clinically integrated network is a physician-led organization that aims to provide value-based care. Collaborative Care of Florida (Orlando). Collaborative Care of Florida is a Track 1 MSSP ACO. It launched in January 2013 and entered into its second agreement in January 2016. e organization is affiliated with Or- lando (Fla.) Health, which has seven acute care hospitals and two affiliat- ed joint venture hospitals. e system comprises more than 20,000 team members and 3,000-plus physicians on staff. e ACO has consistently earned above 90 percent quality rating. Deaconess Care Integration (Evansville, Ind.). Deaconess Care Integra- tion participates in the Next Generation ACO Model, aiming to reduce expenditures while improving outcomes for Medicare fee-for-service beneficiaries. During the 2017 performance year, its second year in the Next Generation model, the ACO reported $16.1 million in shared sav- ings. Evansville-based Deaconess Health System, which includes more than 30 care sites and serves patients in Indiana, Illinois and Kentucky, formed the ACO. Delaware Valley ACO (Radnor, Pa.). Delaware Valley ACO is a Track 1 MSSP organization that launched in January 2014 and began its sec- ond agreement period in January 2017. It serves patients in Pennsylva- nia and New Jersey. e ACO has several participants, including Phil- adelphia-based omas Jefferson University Hospitals and Main Line HealthCare in Newtown Square, Pa. DMH Health Network (Decatur, Ill.). DMH Health Network launched in January 2018 and is a Track 1 MSSP ACO. e ACO is affiliated with Decatur Memorial Hospital, a 300-bed nonprofit hospital with more than 30 satellite offices in central Illinois. e ACO is in its first reporting period. Einstein Care Partners (Philadelphia). Formed in June 2017, Einstein Care Partners is a Track 1 MSSP ACO serving patients in Philadelphia. e ACO is comprised of primary care practices of Philadelphia-based Ein- stein Community Health Associates, Einstein Physicians Philadelphia and Einstein Physicians Montgomery in East Norriton, Pa. e primary care physicians care for around 12,000 beneficiaries through the ACO and serve 60,000 beneficiaries in total under population health payment contracts. Essentia Health (Duluth, Minn.). Essentia Health is a Track 3 MSSP ACO that serves patients in Minnesota, North Dakota and Wisconsin. It launched in July 2012 and entered into its second agreement period in January 2016. Essentia Health Virginia (Minn.), as well as several criti- cal access hospitals and medical groups participate in the ACO, which reported $2.8 million in shared savings for the 2017 performance year. It distributed 40 percent of the savings to participants.
