Three things to know: 1. The U.N.'s Interagency Coordination Group on Antimicrobial Resistance compiled the re- port, which aims to create a sense of urgency in the fight against drug resistance. 2. The group's projection marks a 1,328 percent increase from the 700,000 people who die glob- ally every year from drug-resistant infections. 3. The report outlines key recommendations to combat antibiotic-resistant infections, which in- clude creating more incentives for drugmakers to develop new antibiotics and limiting unnec- essary antibiotic use. n Facility type affects antibiotic stewardship guideline adherence By Anuja Vaidya A study published in Antimicrobial Resistance & Infection Control found that lower complexity facilities continue antibiotic therapy longer than guidelines recommend. Researchers conducted a retrospective cohort study of patients at Department of Veterans Affairs facilities who underwent an outpatient surgical procedure from Oct. 1, 2015 to Sept. 30, 2017. e patients underwent a procedure in one of the five following specialties: general surgery; urology; ophthalmology; ear, nose and throat; and orthopedics. Researchers studied the association between facility com- plexity and proportion of surgeries not compliant with the guideline to discontin- ue antimicrobials within 24 hours of incision closure. Of 153,097 outpatient surgeries, 5 percent involved antibiotic therapy lasting more than 24 hours aer surgery. Lower complexity ambulatory surgery centers and hospital outpatient departments had higher odds of prolonged antibiotic therapy as compared to complex hospitals. Additionally, researchers found genitourinary surgeries, such as cystoscopies and cystoureteroscopy with lithotripsy procedures, had the highest rates of prolonged antibiotic therapy. Hernia repair, cataract surgeries and laparoscopic cholecystec- tomies had the lowest rates.
